5, certification: approved, the production of radiation safety permit; If no license is granted, a Written Decision of no license shall be issued.

What information is required for radiation safety permit processing

1. Application report for Radiation Safety Permit (unit document);

2. The application form for Radiation Safety Permit is made in triplicate. It must be filled in carefully in accordance with the requirements of the "Form Filling Instructions" of the Radiation Safety Permit, and must not be altered;

3, the following materials in triplicate, stamped with the official seal of the unit:

1) Copy of business license of enterprise legal person, certificate of legal person of institution or practice license of medical institution;

2) Copy of the ID card of the legal representative;

3) Environmental impact assessment approval documents for radiation projects;

4) A detailed list of existing and proposed new radioisotope and radiation installations. The following should be included:

Ray device details: device name, specifications and models, ray types, uses, sources, etc.

5) Photocopy of the radiation safety Training Certificate obtained by the radiation safety and protection professional knowledge training organized by the environmental protection department (the State Administration) or the radiation Safety Training Examination Certificate (the Provincial radiation environment Supervision and Management Station).

6) Photocopy of the Radiation Safety Training and Assessment Certificate for the person in charge of the workplace;
